NO EXTRA GUEST Allowed During Your Appointment.

CONSULTATION

DEPOSITS
A non-refundable deposit of 20% of your total price is required for booking an appointment. 
This deposit goes towards the full payment of Service.
Deposits are Non-Transferable to another Client, Deposit can only be used for the month of scheduled appointment.

LATENESS
Please arrive on time for an appointment, there is a grace period of 10 minutes.
Clients arriving 15 minutes late will be charged a $15.00 fee.  Clients arriving over 30 minutes must reschedule their appointment.

CANCELLATIONS
Please notify us 24 hours before your scheduled appointment of Cancelation or to Reschedule.

NO SHOW
For a no show up Client who wants to make another appointment,­­ 25% of Fee will be added to a new appointment.

SERVICE GURANTEE OR REFUND
If a Client is not satisfied, please contact us Within 24 hrs after service to make any adjustments.
Payment is Non-Refundable after Service.

HEALTH HISTORY
Clients must notify and inform LOCSDOC of any allergy or health condition which may affect treatment of their service.

RIGHT TO REFUSE SERVICE
LOCSDOC reserves the right to refuse service if a Client is intoxicated or behaves inappropriately.
